JG Alternative Healthcare is required by law to protect personal information and comply with the Health Records Act 2001, further information can be obtained from the Victorian Government Health Information website .

We collect information that is necessary and relevant in order to provide you with healthcare and treatment, and manage our clinic.

This may include; Your name, address, date of birth, gender, health information, family history, contact details, health fund details, Medicare numbers, or any other personally identifiable information provided when completing our Patient History/Consent Form or when using our online booking system.

We collect your personal information in the following ways; In person, website visits, send us an email or SMS, over the phone, make an online appointment, or communicating and connecting with us using social media.

Personal information may also be collected from other sources. This may include information from your guardian or responsible person, other persons involved such as healthcare providers, specialists, allied health professionals, hospitals, community health services and pathology, diagnostic imaging services, your health fund, Medicare, or the Department of Veterans’ Affairs (as necessary).

This information may be collected by our Practitioners and/or Administration Staff. Only staff who need to see your personal information will have access to it. 

By signing an Intake & Consent Form at your initial appointment, and using our Website, you consent to JG Alternative Healthcare collecting and using of your personal information as described in this Privacy Policy.
